ELIGIBILITY

All Regular, U.S. NesCARE benefits-eligible employees (Hourly regularly scheduled to work 30+ hours or Salaried working 20+ hours, as defined in the NesCARE SPD).

Required: Register with Personify Health

You are REQUIRED to register for a Personify Health account in order to be eligible to receive any Healthy Rewards credit.

Please call the CareFirst customer service line at 1-833-502-9928 and select option #3 between 8 a.m. and 8 p.m. Eastern Time, Monday-Friday.

*You must be an active U.S. NesCARE benefits-eligible employee (Hourly regularly scheduled to work 30+ hours or Salaried working 20+ hours, as defined in the NesCARE SPD) with a first day of work on or before October 18, 2025. You and your spouse/domestic partner must be enrolled in a Nestlé medical plan in 2025, and you and your spouse/domestic partner must have registered for your Personify Health account by November 30, 2025. Spouses/domestic partners who are benefits-eligible must be on record with the Nestlé HR Service Center by October 18, 2025. Healthy Rewards are divided per paycheck throughout 2026. If your employment ends during 2026, your Healthy Rewards credit will cease. Participants with a Personify Health account with steps processed by Personify Health by October 31, 2025, should start receiving Healthy Rewards in January 2026. Participants with a Personify Health account with steps processed by Personify Health no later than November 30, 2025, should start receiving Healthy Rewards in February 2026. Healthy Rewards deadline is November 30, 2025, 11:59 pm EST.
