We know wellbeing isn’t one-size-fits-all. We think about wellbeing holistically — it includes your emotional, physical, financial and social wellness. That’s why we’re giving you options to personalize your Healthy Rewards with Personify Health — so you can make it meaningful to you.
All Regular, U.S. NesCARE benefits-eligible employees (Hourly regularly scheduled to work 30+ hours or Salaried working 20+ hours, as defined in the NesCARE SPD).

Choose from one of the options below to get your biometric screening before November 30, 2025. If you have already had a screening with your health care provider/primary care physician as of December 1, 2024, you can upload your results on Personify Health by utilizing the Primary Care Provider form found within your Personify Health portal.

— Talk to your provider beforehand about fasting guidelines. Take a copy of the health screening form from Personify Health to your appointment. Screening results from doctor visits/health care providers will only be accepted from December 1, 2024, to November 30, 2025, due to program deadline dates. You will need to scan or take a picture of the completed form and upload it directly to your Personify Health account. Log in to Personify Health to register for a Personify Health account and to confirm your participation.

Complete the Health Check Survey online between January 1, 2025, and November 30, 2025, to understand more about your health and get personalized suggestions to help with your overall wellbeing.

Complete a healthcare appointment for medical, mental, dental or vision appointment between December 1, 2024, and November 30, 2025. You will need to manually confirm completion in your online Personify Health account under Rewards.
Up your financial game by taking a Financial Finesse class between January 1, 2025, and November 30, 2025. Visit Financial Finesse to take a class.
You must log in to Personify Health to manually confirm your participation in at least one class to earn your reward.

Please call the CareFirst customer service line at 1-833-502-9928 and select option #3 between 8 a.m. and 8 p.m. Eastern Time, Monday-Friday.
* You must be an active U.S. NesCARE benefits-eligible employee (Hourly regularly scheduled to work 30+ hours or Salaried working 20+ hours, as defined in the NesCARE SPD) with a first day of work on or before October 20, 2025, you and your spouse/domestic partner must be enrolled in a Nestlé medical plan in 2026, and you and your spouse/domestic partner must have registered for your Personify Health account by November 30, 2025. Spouses/domestic partners who are benefits-eligible must be on record with the Nestle HR Service Center by October 20, 2025. Healthy Rewards are divided per paycheck throughout 2026. If your employment ends during 2026, your Healthy Rewards credit will cease. Participants with a Personify Health account with steps processed by Personify Health by October 31, 2025, should start receiving Healthy Rewards in January 2026. Participants with a Personify Health account with steps processed by Personify Health no later than November 30, 2025, should start receiving Healthy Rewards in February 2026. Healthy Rewards deadline is November 30, 2025, 11:59pm EST.
