A qualified family status change is an event that affects your benefits coverage or the coverage of your dependents, such as:
• Significant change (addition, loss or rate change) in your spouse’s/domestic partner’s benefit coverage
Outside of an enrollment period, you cannot change your benefits unless you experience a qualified family status change (election change event).
• Significant change (addition, loss or rate change) in your spouse’s/domestic partner’s benefit coverage
If you have a baby and/or adopt a child and notify Nestlé of the change within 30 days, your child’s coverage will take effect on the birth and/or adoption date.
If you enroll the child after the 30 days from the date of birth or adoption but within 60 days, you will have a gap in coverage if you do not currently cover other dependent children on your benefits.
Remember to submit dependent verification forms within 60 days of enrolling any dependent under your benefits coverage.

If you have a qualified family status change, log in to My Benefits and click on the “Change My Benefits” link or call the Nestlé Benefits Service Center — as soon as possible, within 60 days — to make any benefit changes.
Generally, your coverage will take effect on the date you make the change online or notify the Nestlé Benefits Service Center about the change.
Any changes you make must be consistent with your event. See your Summary Plan Description (SPD) on My Benefits for details.
